Format	4 Man Scramble, Minimum 3 Drives Per Player
	Players will tee off on number 1 and 10 with assigned tee times. Please check www.pmlmgc.com for tee times. Tee times will be posted 48 hours prior to the tournament.

Guests	You may bring a guest. Your Non-Member guest must have a current Golf Handicap Index Number (GHIN) listed in the space provided below.
